Basically this drawing is the aftermath of the fight Dexter and Mason had, where Mason had to sacrifice his morals to kill his own brother to protect Xavi (Aka breaking right horn) however Mason made Xavi promise to kill him afterwards but she didnt have the heart too, instead she called for help and Mason was taken away in the ambulance to a hospital. Xavi obviously thought she was saving him but didnt realize that Mason is a cold blooded killer like his brother and now he's stuck to live that way instead of letting him die along with Dexter, and he's basically telling Xavi in the hospital that shes basically a leech sucking the life out of him cuz she cant let him go.
